fix(vision-bridge): lock unsupported models on 400 - #18271
Vision Bridge model-scoped HTTP 400 responses now lock only the rejected provider/connection/model.
Fixed-model selection checks existing lockouts before reuse, allowing fallback to continue and subsequent requests to skip the rejected model.
Root cause: the Vision Bridge retry loop did not feed model-scoped 400 responses into the existing lockout mechanism, while fixed-model selection bypassed lockout checks.

🟡 Changes recommended
Active routing issues can lock healthy sibling models after an unsupported-model error and exclude a compatible-node model despite another healthy connection.
This PR addresses #18268 by connecting Vision Bridge’s unsupported-model 400 handling to model lockouts, allowing fallback and later requests to avoid rejected models.
Changes:
Records a one-hour lock when a model-scoped 400 includes a selected connection ID.
Checks lockouts before reusing a fixed vision model.
Adds a regression test for fallback and subsequent-request skipping, plus a changelog entry.
Validation: No commands were run during this review. The PR reports 13/13 focused tests passing; the required npm run test:coverage result is not provided.
…y under the complexity ratchet
Behavior is unchanged: getBestVisionModel delegates the credential and lockout decision for a fixed model to a small helper.

Vision requests now continue to a fallback model when a selected model returns an unsupported-model error, rather than retrying the rejected option.
Rejected models are skipped for the affected connection for up to one hour, helping subsequent requests avoid the same failure.
Fixed-model selections now account for credential failures and available serving connections before proceeding.
Vision Bridge now locks a provider, connection, and model after a qualifying model-scoped 400 response. Fixed-model selection checks provider-prefixed models against lockout state. A regression test verifies fallback and subsequent avoidance of the locked model.

Fixed-model selection checks provider-prefixed models against lockout state. The regression test verifies fallback after a model-scoped 400 and confirms a later call avoids the locked model. The changelog records this behavior.
Vision Bridge now locks a model after a model-scoped 400, but the lock can be broader than intended for some providers. Fixed-model selection can also misjudge availability for key-restricted callers and compatible-node providers. These edge cases can block or mis-route image descriptions for up to an hour, so fix them before merging.
Security architecture risk: Moderate · up to 451e7
A rejection for one configured option can temporarily suppress other healthy options on the same account. This can affect later requests, although the effect remains account-scoped and time-limited. Existing access restrictions remain enforced.
Retained concerns
Medium · reliability · inferred: The new model-scoped 400 handler calls the quota-family-aware lockModel API. For recognized Antigravity models, one unsupported-model rejection can therefore quarantine healthy sibling models in the same family on that connection, rather than only the rejected model. Later Vision Bridge selections consult that family lock, extending failure impact across requests for the one-hour cooldown unless cleared earlier. The existing model-scoped 400 helper instead uses exact-model locking for Antigravity. Provider and connection isolation remain intact, but model-level failure containment is weakened by this new caller.
Security review details
Security Blast Radius
inferred — Lock identity contains canonical provider, connection, and model scope, but no caller-key identity. Consequently, callers sharing that connection and in-process state can inherit the same quarantine. The identified broadening is within an Antigravity family on one connection, not a demonstrated cross-provider or cross-connection write.
Security Findings and Attack Paths
inferred — The supported concern is availability amplification: an upstream model-specific rejection becomes shared family-level quarantine. A deliberate attacker-controlled trigger was not established; reaching the new write requires a classified HTTP 400, a selected connection header, and a provider-prefixed attempted model.
Trust Boundaries and Controls
observed — For internal chat routing, the selected-connection response header is set or overwritten from the actual serving credentials, including terminal error responses. Key-scoped describe requests are forced through the internal route and carry the caller key identifier alongside the internal bearer; credential checks remain narrowed by the key's model and connection policy.
Resilience and Maintainability Implications
observed — The availability gate preserves a model when any known candidate connection is unlocked and excludes it when all candidates are locked. Missing connection knowledge or an unavailable credential store allows selection to continue; that availability decision does not replace the separate scoped credential policy.

Use an exact lock for Antigravity model-scoped 400 responses.
The vision bridge calls lockModel directly. For Antigravity, this can lock the rejected model's entire quota family for one hour. Keep the existing quota-family behavior for Codex.

Scope the fixed-model lockout check to the caller's permitted connections.
isFixedModelSelectable checks the key-scoped credential result, but isModelUsableGivenLockouts does not receive the key's allowed connection IDs. getUsableConnectionsForModel returns every usable connection for the provider, and collectLockoutConnectionIds adds every locked connection. An unlocked connection outside the key's allowed set can therefore make the check return true while every permitted connection is locked. The bridge then keeps the rejected fixed model. The describe request goes through the key-scoped self-loop and fails on the locked permitted connection, so the fallback path does not run.
Pass the key's allowed connection IDs into the lockout check, and limit the candidate IDs to that set. Apply the same filter to the lockout IDs as to the usable connections. Leave unrestricted keys on the current path.

Resolve compatible-node IDs in the lockout connection lookup.
For a public-prefix model, hasUsableCredentialsForModel checks the mapped node ID. getUsableConnectionsForModel checks only the literal prefix. A 400 lock on one node connection therefore leaves the lockout check with only that locked connection. isFixedModelSelectable then rejects the fixed model for one hour, even when another connection under the node ID is usable.
Query each provider ID from resolveProviderCredentialIds(provider, prefixToNode) in getUsableConnectionsForModel, as hasUsableCredentialsForModel already does. Merge the usable connections from those queries before the lockout check runs.
