fix(plugin): refresh the served catalog on the configured TTL instead of publishing once at install - #18235
The served catalog stayed frozen on an earlier calculation even though config and gateway had moved, so new branches stayed missing until a service restart. The plugin published the catalog once at install and never refreshed it, so the configured cache TTL had no effect. Now the catalog refreshes in the background on that TTL, and the served catalog follows the gateway without a restart.

@omniroute/opencode-plugin-v2/tests/catalog-freshness.test.ts — background refresh serves new gateway entries, unchanged catalogs skip reload, refreshes never overlap, one loop per provider id, 60 second floor, a failed refresh keeps the last known catalog, and the stale memory entry is dropped on credential change, plus gateway change, snapshot coherence and branch reactivation without a restart.
Coverage Notes
N/A — no file under the repository's src/, open-sse/, electron/, or bin/ is changed; the plugin lives under @omniroute/opencode-plugin-v2/.
Reviewer Notes
Behavior: the served catalog goes from frozen after install to following the gateway within modelCacheTtlMs (five minutes by default), plus one fetch; a failed refresh keeps the last known catalog, and a change that only touches enrichment (names or prices) may not trigger a host reload.
Follow-up: the cast on the global loop registry stays confined to one line and could be replaced by a typed guard later; the branch-name charset comes from the worktree and is not addressed here.
The available model catalog now refreshes in the background at the configured interval, with a minimum interval of 60 seconds. Gateway changes appear without requiring a service restart.
The catalog can still load from a saved snapshot on startup.
Bug Fixes
Prevented overlapping refreshes and duplicate refresh loops. Unchanged or unsuccessful refreshes no longer trigger a reload.
The OpenCode plugin now refreshes its served catalog in the background, with a minimum interval of 60 seconds. Setup manages refresh-loop cleanup and credential-related cache entries. Tests cover catalog updates, refresh failures, and snapshot consistency.

Summary
Refresh interval and loop @omniroute/opencode-plugin-v2/src/cache.ts, @omniroute/opencode-plugin-v2/src/refresh-loop.ts
The plugin defines a 60-second minimum refresh interval. The loop skips overlapping refreshes, logs failures, and returns a stop function.
Setup starts and replaces refresh loops by provider ID. Forced refreshes bypass fresh cache entries. Stopped setups skip snapshot writes, optional-source upgrades, and reloads. Changed credentials cause the previous cache and in-flight entries to be removed.
Freshness validation and documentation @omniroute/opencode-plugin-v2/tests/catalog-freshness.test.ts, @omniroute/opencode-plugin-v2/README.md, changelog.d/fixes/18235-opencode-plugin-v2-stale-served-catalog.md
Tests cover timed and failed refreshes, loop replacement, credential changes, and catalog and snapshot consistency. The README and changelog describe the refresh interval and cold-start snapshot behavior.
The catalog now refreshes in the background, but replacing a plugin installation can briefly let the old installation reload the catalog, and an extremely long TTL setting can cause continuous gateway requests. Stale cleanup can also leave refresh loops that later installs cannot stop. Fix these before merging.
Security architecture risk: Moderate · up to b19cb
Background refresh adds two failure-containment risks: sufficiently large configured TTLs can trigger rapid repeated requests, and delayed cleanup of an older installation can leave a newer refresh loop untracked. Existing credential isolation limits cross-credential catalog reuse, but does not resolve these lifecycle issues.
Retained concerns
Medium · reliability · inferred: The newly autonomous refresh schedule accepts TTLs above Node's maximum timer delay. For example, a 30-day TTL passes validation and the lower-bound clamp, but Node normalizes delays above 2,147,483,647 ms to 1 ms. Each available tick forces refresh past the normal TTL cache check. Non-overlap prevents simultaneous callbacks, but healthy responses can still produce continuous authenticated gateway requests and snapshot writes. This turns a previously cache-only configuration value into a request-rate and failure-containment hazard.
Medium · reliability · inferred: Cleanup is not ownership-safe across replacement. After installation B replaces A, invoking A's returned cleanup again deletes B's registry entry without stopping B's timer. A subsequent installation C therefore cannot discover and stop B. B can continue authenticated requests, shared-path snapshot writes, and reload requests using its captured configuration alongside C. This weakens containment of superseded credentials and gateway configuration. The unsafe transition is supported by source; whether the host produces this cleanup ordering remains unresolved.
Security review details
Security Blast Radius
inferred — The supported direct exposure is the plugin host, provider-scoped catalog state and snapshot file, and configured gateway endpoints accessible with existing credentials. Rapid refresh or escaped lifecycle ownership can increase authenticated traffic and persistence activity within that scope; broader cross-tenant or cross-service authority is not established.
Security Findings and Attack Paths
inferred — The timer-overflow path requires a large TTL supplied through host plugin options; the inspected path does not derive that value from gateway responses. The orphan-loop path requires replacement followed by delayed cleanup of an older installation. These are bounded configuration and lifecycle conditions, not demonstrated unauthenticated remote attack paths.
Trust Boundaries and Controls
observed — Provider identifiers are restricted before entering filesystem paths. Snapshot writes use private directory/file modes, unique temporary names, a size cap, and atomic rename. Disk identity matching remains the control separating snapshots belonging to different gateway and credential tuples; atomic rename itself does not establish lifecycle ownership.
Resilience and Maintainability Implications
inferred — Stopped checks limit obsolete work but do not cancel an awaited refresh or fence a snapshot write already started before cleanup. An older write can therefore finish after replacement. The shared-path writer predates this PR, so this is not presented as a newly introduced credential-isolation vulnerability; periodic refresh makes its ordering relevant to ongoing lifecycle containment.

Stop the previous installation before the replacement publishes.
existing.stop() correctly sets the previous setup's stopped flag. However, the replacement calls await refreshAndPublish() before it invokes existing.stop(). An old refresh that has already passed its stop guard can therefore reach ctx.provider.reload() while the replacement is publishing. Move the existing-loop replacement, or otherwise invoke the old cleanup, before the replacement's initial refresh.

Bound the interval before passing it to setInterval.
The options schema accepts a TTL above 2,147,483,647 ms. On the supported Node runtime, setInterval converts that delay to 1 ms. A configuration such as 30 days therefore causes repeated gateway requests instead of infrequent refreshes. Reject unsupported TTLs or schedule long periods in bounded segments. (r2.nodejs.org)

Delete only this installation’s registry entry.
If the host calls an older setup’s cleanup after a replacement is registered, registry.delete(X) removes the newer entry. The newer timer keeps running, but a subsequent installation cannot find and stop it. Repeated replacements can therefore stack refresh loops. Delete the entry only when it still belongs to this setup.

Wait for refresh completion instead of waiting 200 ms.
mock.timers.tick() triggers the interval, but the interval callback does not return the refresh promise. The fixed setTimeout therefore cannot establish that publication and reload have finished. A slow CI run can fail this assertion even when the refresh is correct. Use a reload completion signal or bounded polling; the other post-tick fixed waits need the same change. (nodejs.org) Based on learnings: “Flag a test that relies on a fixed delay instead of synchronizing with the asynchronous background outcome.”
