Docs-only PR for the 0.28.0 release. Merge after #1686 (Claude Code mod on by default). This branch is main + #1686 merged + one docs commit, so until #1686 lands the diff also shows #1686's commits. Only the last commit (docs: 0.28.0 …) belongs to this PR.

What changes

  • Claude Code mod (guides/claude-code.md, commands/plan-review.md, getting-started/quickstart.md, getting-started/installation.md, apps/hook/README.md, README.md). The mod is on by default on Claude Code 2.1.287+ in the interactive terminal. Plan review, code review, annotate and last don't make Claude wait. The decision arrives later as a message. A revised plan updates the same tab. After approval, Claude calls ExitPlanMode once more and runs the exact approved text. Claude can open Plannotator with the plannotator tool. Older Claude Code, -p/SDK and Windows keep the classic flow. Opt-out: PLANNOTATOR_CLAUDE_MOD=0 / { "claudeCodeMod": false }. The pages also state the known limit: if you leave plan mode before approving, Claude can start editing. The "Long reviews" Bash timeout note now says it applies only without the mod.
  • Env/config reference: adds PLANNOTATOR_CLAUDE_MOD, PLANNOTATOR_MOD_DEBUG and the claudeCodeMod key.
  • Ask this session (guides/ai-features.md, commands/code-review.md, Pi and OpenCode READMEs, guides/opencode.md). When Plannotator is opened from Claude Code (mod), Pi or OpenCode 2, the session is the only Ask AI option. Covers busy handling (Ask when it finishes / Interrupt and ask now), the OpenCode 2 plan-review quick answer, when you still pick a provider, and that Review Agents / Code Tour / Guided Review keep their own models.
  • Plugin update steps (docs, READMEs, install.sh / install.ps1 / install.cmd closing banner; echo text only). /plugin marketplace update alone does not update an installed plugin. Verified against Claude Code 2.1.289:
    • claude plugin marketplace update plannotator, then claude plugin update plannotator@plannotator, then restart (or /reload-plugins).
    • The /plugin slash command has no update subcommand: /plugin update … just opens the menu. Inside Claude Code the path is /plugin marketplace update plannotator, then /plugin → Installed → plannotator → Update now.
  • Pi: pi update --extensions (a plain pi update updates only Pi; checked against pi update --help, Pi 0.85.1). Also: Esc no longer cancels a plan review (leave plan mode to abandon it), and Ask this session.
  • OpenCode: re-running the installer now also clears the OpenCode 2 plugin cache. I left out opencode2 plugin update because I could not confirm the command exists.
  • Troubleshooting: new entry "Claude Code still waits for my review".

Checks

  • bun test scripts/install.test.ts (plus the other tests that read README/docs paths): pass. No test pins the old banner lines.
  • bun run build:marketing: builds, and the new anchors (#the-plannotator-mod, #turning-the-mod-off, #ask-this-session, #updating) resolve.

…ct plugin update steps

Claude Code docs describe the Plannotator mod (on by default on 2.1.287+,
non-blocking plan review/review/annotate/last, revisions in the same tab,
two-hop approval, the plannotator tool, opt-out, the leave-plan-mode
limit). Env and config references add PLANNOTATOR_CLAUDE_MOD,
PLANNOTATOR_MOD_DEBUG and claudeCodeMod. Ask AI docs add Ask this session.

Plugin update steps: /plugin marketplace update alone does not update an
installed plugin. Docs, READMEs and the install.sh/ps1/cmd closing banner
now give claude plugin marketplace update plannotator + claude plugin
update plannotator@plannotator + restart (verified against Claude Code
2.1.289; the /plugin slash command has no update subcommand, so the
in-session path is /plugin > Installed > plannotator > Update now).

Pi: pi update --extensions, Esc no longer cancels a plan review, Ask this
session. OpenCode: the installer clears the OpenCode 2 plugin cache too.
…eload-plugins; plan-mode command takes no path
