Why

Reviewing the dev→main PR with Codex surfaced the H3 SessionStart line:

The hook was accurate — the ledger was stale: waves 1+2 shipped 2026-07-22 (PRs #2625/#2626/#2628/#2629, per-group SHIP reviews, validations reproduced by the invoking orchestrator) but their evidence and criteria were never recorded in WISH.md, and status never advanced past APPROVED.

What

• Status APPROVED → IN_PROGRESS (blocked=true persists by design: it derives from codex-plugin-update-handoff's WISH→SHIPPED, which remains gated on the operator's homolog ritual).
• 41 evidence-backed criteria ticked (5 → 46/67): Groups A–D ACs + global criteria owned by merged groups. Deliberately NOT ticked: the two live codex mcp get genie --json operator proofs (AC3 ritual — output never captured), Group F/G criteria, all post-merge QA rows.
• Evidence block appended: waves 1+2 per-group PRs and reproduced validations; Group E's actual merge commit (4be6917f); the seven live-QA defects found and fixed same-day (fix(setup): consume journal-quarantine permits (Group E live-QA regression) #2631–fix(delivery): publish the delivery record for absent-N (fresh-host setup gate) #2634, fix(delivery): republish a stale record when a verified delivery lands on a current generation #2636); and the open fix(doctor): teach plugin checks the post-A manifest contract #2633-deferred follow-up (pre-A route-arm retirement in the probe) now durably tracked instead of living only in a PR body.

Verification

Pre-commit adversarial pass via the pm-ledger-verify workflow (3 independent lenses): 0 must-fix; its 4 advisory findings — defect miscount (six→seven), the untracked #2633 follow-up, and the unrecorded E merge — are incorporated. bun run wishes:lint OK (64 files, 0 broken links).

Docs-only: no code, no tests, no behavior.
