fix(opencode): avoid nullable webfetch format schema

• remove redundant optional wrapping from webfetch.format so its emitted tool schema no longer advertises null
• preserve markdown defaulting for omitted and undefined format inputs
• add regression coverage for the non-nullable wire schema and default decoding behavior
