• post-pr-monitor の re-push 時ポーリング問題: re-push 後に CodeRabbit の新レビューを待たずに旧状態で即判定する問題
• analyze-coderabbit.md と fix.md の read-only zone 定義の齟齬: analyze が applicable と判断した finding を fix が修正できない misdirected パターン
