Implements D + E from #391.

The problem

pnpm audit --audit-level=high is a blocking gate; minimumReleaseAge: 4320 refuses anything published in the last 72 hours. When an advisory's only patch is younger than that, the two rules demand opposite things and every open PR goes red over a transitive dependency nobody touched. The escape hatch (force the version in overrides, waive the cooldown for it) worked but was undocumented and permanent by default — nothing made the expiry observable, so entries outlived their own stated reason.

D — the runbook

CONTRIBUTING.md gains the actual procedure: force the patched version scoped to its major, add the cooldown exclusion if the patch is still inside 72h, annotate both with a review date, confirm the audit is clean, commit the lockfile. It also says plainly that a red gate from this cause is not your PR's fault, and that pnpm all does not run the conformance checks (CI does). pnpm-workspace.yaml states the contract at the top, where someone editing it will see it, and CLAUDE.md carries the rule so other sessions inherit it.

E — the gate

New bypass-expiry conformance check. Every entry in overrides, minimumReleaseAgeExclude and auditConfig.ignoreGhsas must carry one of:

# bestax:review 2026-11-13 — drop once the ajv chain resolves to >=3.1.5 itself
# bestax:permanent — why this is standing policy, not debt

It fails on a missing annotation and once a review date arrives. The missing-annotation half is what gives it teeth — without it a new bypass just omits the marker and the check is decorative.

Two scope decisions worth flagging for review:

  • All three lists, not just cooldown exclusions as [Refactor] The audit gate + 3-day cooldown structurally guarantee red CI when a patch is fresher than 72h #391 scoped it. That list holds exactly one entry today (prettier, permanent); overrides holds fourteen. Policing only the former would have shipped a check that guards an empty surface.
  • Hand-rolled line scan (scripts/lib/bypass-annotations.mjs), mirroring the existing parseWorkspacePackages. The repo declares no YAML parser anywhere, and adding one to police the cooldown would itself be subject to the cooldown.

Existing entries are dated 2026-11-13 — all reviewed today, one quarterly sweep rather than invented staggered dates. Expired entries in a list report as one grouped violation, so a batch coming due is a single chore.

Honest limits

  • This does not stop the red gate. A fresh advisory still reds every open PR; D makes the fix predictable and E makes the cleanup non-optional. Only option C (audit warns on PRs, blocks on a schedule) removes the unrelated-PR breakage, and that is deliberately deferred.
  • E is mildly self-undermining: a date-triggered check fails on a schedule, on PRs unrelated to the diff, which is the same class of problem [Refactor] The audit gate + 3-day cooldown structurally guarantee red CI when a patch is fresher than 72h #391 is about. The issue concedes this in its Risks section. Mitigation is entirely in the message quality and the grouped reporting.
  • No .github/ changes — check:conformance already runs in the required Build and Test job.

Testing

Ran against the real file, not just unit tests:

  • --only=bypass-expiry clean on the annotated file (18 entries parsed: 16 dated, 2 permanent).
  • Back-dated one override → exit 1, message names the entry, line, date and the prune loop. Back-dated a second → confirmed the two report as one grouped violation.
  • Stripped an annotation → correct missing-annotation message. This case also proves the comment-association fix: fast-uri@3 did not inherit the preceding block's date.
  • prettier never trips in any state.
  • Full pnpm check:conformance green (14/14, no regression), pnpm lint clean, node --test "scripts/*.test.mjs" 42/42.
  • pnpm install --frozen-lockfile clean and pnpm audit --audit-level=high still exits 0 — annotations are comments, so the lockfile must not move, and it doesn't.

How can I continue?

After more reviews become available, a review can be triggered using the @coderabbitai review command as a PR comment. Alternatively, push new commits to this PR.

To avoid repeated limits, reduce automatic review volume by pausing incremental auto-reviews earlier, using label-based review opt-in, excluding WIP or generated PR titles, or requesting reviews manually when the PR is ready. If your team needs uninterrupted high-volume reviews, an organization admin can enable usage-based reviews.

How do review limits work?

CodeRabbit enforces per-developer PR review limits for each organization. Most developers receive the normal plan review availability.

For paid Pro and Pro+ PR reviews, CodeRabbit uses adaptive limits for sustained high-volume activity. When a developer's recent PR review activity reaches the 95th percentile or higher among CodeRabbit users, additional reviews become available more gradually as earlier reviews age out of the rolling window.

Merge Risk: 🟡 Moderate · up to 4cff2

This PR adds review dates and expiry enforcement for dependency bypasses, but the current gate can silently stop policing a bypass list if its block is renamed or moved, and malformed entries can produce misleading failures; the runbook also prescribes an override form that conflicts with established single-floor usage. These are actionable merge-readiness risks that should be fixed or explicitly accepted before merge.

All three review findings applied — thanks, every one was a genuine fail-open where the gate would go green while a bypass went unpoliced.

  • Typo'd dates (9999-99-99, or a rolled-over 2026-02-30): validated as real calendar dates now. Lexicographic comparison meant a malformed date sorted after every real one and never came due, so a typo made a bypass permanent. Both markers on one entry, and a marker with no reason, are also errors rather than silent passes; malformed entries report as malformed rather than unannotated so the message names the right fix.
  • Early block termination: blocks now end only at a real dedent, list items with inline comments parse, and an unrecognised indented line is reported instead of silently dropping that entry and every one below it.
  • Marker inheritance (a7b9afa): a block now belongs to exactly one entry and is consumed when read, so all 18 bypasses carry their own marker. Beyond being safer, it's more correct — brace-expansion@1 can become droppable while @5 is still load-bearing, so each line is its own decision with its own date. The shared-block test inverted into the regression guard for the appended-entry case.

Validating the dates also surfaced a crash neither of us flagged: toISOString() throws on an Invalid Date rather than returning a mismatch, so a typo would have taken the whole conformance run down with a stack trace instead of printing a violation.

Verified by driving the real file, not just the unit tests: appending 'sneaky@1' directly under js-yaml@4 with no comment is now caught by name and line; a typo'd date reports cleanly; an inline comment on a list item still parses all 18 entries with zero problems. Conformance 14/14, root tests 49/49, lint and format clean, --frozen-lockfile and pnpm audit unchanged.

  • match() retains only the first occurrence of each marker type, so a block with two bestax:review markers is accepted. If someone adds a new future marker above an old due marker instead of replacing it, the gate silently ignores the contradictory expiry despite the one-annotation contract. Count all marker occurrences, reject totals other than one, and cover duplicate review/permanent markers in the tests.

  • This instruction should require a version-qualified exclusion. Excluding the bare package disables the cooldown for every release until the review date; combined with the range override above, a later lockfile refresh can admit a newer, unreviewed release published inside the 72-hour window. Add only the known patched release to minimumReleaseAgeExclude.
2. If the patch is still inside the cooldown, add the package to `minimumReleaseAgeExclude` too.

  • This test header contradicts the implementation and the assertions below: the parser explicitly consumes the comment block after the first entry, and every entry requires its own marker. Describing shared comment blocks and saying they cannot be consumed at the first entry obscures the invariant these tests actually protect.

Picked up all three suppressed comments from the last pass in 2a92e11 — all three were right.

  • Stacked markers of the same kind. match() without /g keeps the first, so adding a fresh date above a stale one instead of replacing it silently dropped the overdue date. Markers are now counted and any total above one is an error, which subsumes the earlier review+permanent case into one rule: exactly one marker, whatever the mix.
  • Bare package name in the runbook. Fair catch, and a security one: excluding fast-uri rather than 'fast-uri@3.1.5' waives the cooldown for every future release of that package, so a later lockfile refresh could admit a different just-published version nobody reviewed. The runbook now requires the version-qualified form, which is also what this repo actually did for undici@7.29.0 before that entry was pruned.
  • Stale test header. Mine — the previous commit replaced the shared-comment-block design and I left the header describing it, so it contradicted both the implementation and the assertions beneath it.

Verified on the real file again: stacking a second bestax:review above nanoid@3 now fails with the entry, line, and the "replace a stale date rather than stacking" fix. Conformance 14/14, root tests 51/51, lint and format clean, lockfile and audit unchanged.

Worth noting for whoever merges: every finding across all four review rounds has been a fail-open in code whose entire purpose is to fail closed. That is a fair signal this check earned the scrutiny, and the reason each fix ships with a regression test naming the exact silent behaviour it prevents.

  • This accepts digits as the required rationale, so # bestax:permanent — 2026-11-13 passes and exempts a bypass forever despite supplying only a date, contrary to the documented “why” contract. Require actual letters in the rationale so a mistaken permanent marker cannot turn a dated exception into a silent standing policy.

Consider staggering the review dates.

Every dated marker in this file uses 2026-11-13. On that day the bypass-expiry check fails, and every open PR goes red until someone edits this file — the same unrelated-PR breakage that #391 describes. Two options keep the gate useful without the cliff: spread dates across the quarter by advisory age, or report expired entries as warnings on pull requests and keep them blocking on main.

All four applied in 75bdd46 — thanks, the block-guard one is the best catch on this PR.

  • Per-block guarding. You're right that the zero-entries guard was theatre: it only fired when every block parsed empty, so renaming one header left that list unpoliced while the other two kept the total nonzero. Exactly the fail-open shape I'd already written a comment about for parser termination, and I still left it in the caller. The parser now reports blocksSeen and the check fails per missing block, naming the escape hatch for a list that was deliberately emptied so the fix isn't "delete the guard".
  • Comment leak past an unparsed line. Also right, and my test was masking it — it asserted only entry names, so after silently reported carries 2 bestax: markers (the wrong defect, hiding its real annotation) and the suite stayed green. An unparsed line now separates a block the way a blank line does, and the test asserts the annotation, not just the name.
  • Override selector. Good catch that the runbook contradicted the committed file: postcss@<8.5.18 and sharp@<0.35.0 are unbounded-lower precisely because those advisories have one patched floor. Step 1 now distinguishes the two cases and says why scoping to today's lockfile major leaves older majors unguarded.
  • Group prose. Fixed; "all reviewed" was left over from when a block could annotate a group.

Verified at runtime, not just in tests: renaming minimumReleaseAgeExclude: in the real file now fails with the block name and the fix, where before it passed clean. Conformance 14/14, root tests 53/53, lint and format clean, lockfile and audit unchanged.

Note I did not run the coderabbit review CLI suggested in the agent hint — I don't pipe remote install scripts to a shell. Happy to re-request a review here instead.

Fixed in 33123f2 — confirmed the fail-open before changing anything:

# bestax:permanent-ish — reason   ->  {"permanent": true, "error": null}

\b matches between permanent and a hyphen, so a near-miss spelling parsed as a valid permanent exemption and disabled expiry for that entry forever. Markers must now open their comment and end at whitespace or end of line, so a typo is not an annotation at all and the unannotated rule catches it. Anchoring also closes the mid-prose case (# … see # bestax:permanent …), which previously only escaped by luck — nothing required the marker to follow the comment's own #.

Regression cases cover permanent-ish, permanently, reviewed, review-by and a mid-prose mention, each asserting it lands as unannotated rather than as a valid or malformed marker — fails closed, and the message tells the author the exact form to write.

Runtime check on the real file: typo'ing the prettier pin's marker to bestax:permanent-ish now fails naming that entry and line, where before it passed clean. Conformance 14/14, root tests 55/55, lint and format clean, lockfile and audit unchanged.

  • This lower bound makes the test fail after four valid bypasses are pruned, even though pruning them is the workflow this change is meant to enforce. Keep only a nonzero guard here (or assert the expected blocksSeen keys separately) so the test detects a parser matching nothing without baking existing dependency debt into the suite.

Two suppressed review comments, both real.

overrides is a YAML mapping, so its empty form is {} — the empty-block path
only accepted [], which is an empty sequence and the wrong collection type.
Pruning the last override to the correct {} was reported as a missing block,
and the diagnostic then told the maintainer to write the wrong YAML. Each
block now declares its own empty literal, the message quotes it, and a test
asserts every literal matches the pattern that accepts it.

SECURITY.md still described cooldown exceptions as carrying an inline
rationale and a removal date, which contradicts the contract this PR
introduces: the marker goes in the comment above the entry, covers all three
bypass lists, and the date is a review-by rather than a removal deadline.

overrides: {} vs overrides: []. Right on the collection type: overrides is a mapping, so its empty form is {}, and I only accepted [] — an empty sequence. So pruning the last override to the correct YAML was reported as a missing block, and the message then told the maintainer to write the wrong type. Each block now declares its own empty literal (overrides: {}, minimumReleaseAgeExclude: [], ignoreGhsas: []), the diagnostic quotes that literal, and there's a test asserting every literal actually matches the pattern that accepts it — so the advice and the parser can't drift apart again.

SECURITY.md contradiction. Also right, and worth more than a doc tidy: it described cooldown exceptions as carrying "an inline rationale and a removal date", which conflicts with this PR on three counts — the marker goes in the comment above the entry rather than inline, it now covers all three bypass lists rather than just the cooldown exclusions, and the date is explicitly a review-by rather than a removal deadline. CONTRIBUTING links that guide directly above the new runbook, so a contributor would have hit both versions in one sitting. Updated to describe the actual contract, including the bestax:permanent form.

I checked whether the same stale wording lived anywhere else: docs/docs/guides/security.md only says prettier is the current exclusion, which is still true and doesn't contradict the contract, so I left it.

Verified the {} path against the real file, not just the unit test: emptying overrides to {} now registers the block as present. Conformance 14/14, tests 63/63, lint and format clean, audit exits 0.

Comment thread scripts/lib/bypass-annotations.mjs Outdated
YAML block sequences may be indentless: items sit at their key's own
indentation rather than deeper. The dedent test read the first such item as
a sibling key and ended the block, so BOTH list-valued bypass lists dropped
out of the parse entirely — no entries, no problems recorded, and blocksSeen
still holding both headers, which left the gate green with every cooldown
exclusion and audit ignore unpoliced.

Blocks now declare whether they are list- or mapping-valued. A sequence item
at the header's own indentation continues a list block; anything else at that
indentation still ends it, so a sibling list like publicHoistPattern is never
absorbed and a mapping block like overrides is unaffected. Both directions
are pinned, including the mapping case.

Fixed in a51e7ce. Confirmed the fail-open first, since this one took out both list blocks at once:

minimumReleaseAgeExclude:
# bestax:permanent — deterministic formatting
- prettier
auditConfig:
  ignoreGhsas:
  # bestax:review 2026-11-13 — quarterly sweep
  - GHSA-aaaa-bbbb-cccc

Before: entries: [], problems: 0, blocksSeen: {minimumReleaseAgeExclude, ignoreGhsas, overrides} — every cooldown exclusion and audit ignore unpoliced, and nothing anywhere said so. The per-block guard couldn't help because the headers were matched; only their contents disappeared.

Blocks now declare whether they're list- or mapping-valued. A sequence item at the header's own indentation continues a list block (and the entry pattern relaxed from \s+ to \s* to match one), while anything else at that indentation still ends it. Both directions pinned: indentless items are parsed with their annotations, a sibling publicHoistPattern list is never absorbed, and overrides — a mapping, where a same-indent line really is a sibling — still ends at one.

Tests 66/66, conformance 14/14, lint and format clean, audit 0.

One note on the shape of this PR, for whoever merges. That is nine review rounds, and every finding has been a fail-open in a gate whose entire purpose is failing closed — three of them (unindented comments, {} vs [], indentless sequences) were valid YAML the hand-rolled scanner did not model. The parser is stricter each round, but the recurrence is itself evidence: line-scanning YAML means the failure mode is "looks fine, checks nothing". If a fourth surface is ever added (#516), that is the moment to weigh a real YAML parse behind a cooldown-aged dependency against continuing to grow this.

  • The >= 15 assertion hardcodes today's bypass count. Once enough temporary entries are pruned—the intended remediation—this test fails even though all three blocks may legitimately be empty and the parser explicitly supports that state. Assert that every BYPASS_BLOCKS key was seen instead; this still catches a silent parser miss without requiring bypass debt to remain.
// A parser that silently matched nothing would pass every assertion below.
  assert.ok(entries.length >= 15, `parsed only ${entries.length} entries`);

The guard asserted at least 15 parsed entries, which hardcoded today's
bypass debt: pruning entries — the entire point of this gate — would have
failed the suite, and the parser explicitly supports all three blocks
standing empty. It now asserts every BYPASS_BLOCKS key was found, which
still catches a parser that silently matched nothing without requiring the
debt to stay.

Applied in 207810b — good catch, and a more interesting one than it looks.

The guard asserted entries.length >= 15, which hardcoded today's bypass debt. So the test would have failed on success: prune enough entries — the entire purpose of this gate — and the suite goes red, even though the parser explicitly supports all three blocks standing empty. A test that penalises the outcome you want is worse than no test, because the cheap fix under time pressure is to delete the assertion rather than lower the number.

It now asserts every BYPASS_BLOCKS key was found in the committed file, which still catches the thing the original was reaching for (a parser that silently matched nothing) without requiring debt to remain.

Verified by simulating the success case rather than reasoning about it: rewrote the real file with the exclusion list pruned to minimumReleaseAgeExclude: [], and all 33 parser tests still pass. Under the old assertion that state would have failed.

Tests 66/66, conformance 14/14, lint and format clean, audit 0. All threads resolved.
