Update XSL Template & Rebuild PDF Man

Update the submodule pointer to the latest version of alan-xsl-fopub,
and rebuild PDF Manual.

The template update removes URL footnotes for links, which were being
added for the benefit of printed documents -- but looked ugly on PDF
docs, which provide active click-able links. (See #64)
