these points are significant and should be addressed quickly/soon, but should not hold up merge of #8443 for the 25 release @regisoc

• Number of active channels should be numeric
• move Artefacted Channels question after Bad channels
• add "Were there any Artefacted channels? " before the textarea to list artefacted channels, and add a rule dependency (see bad channels example)
  Task rules need to be looked at:
• if any task was not started or not_answered, its Number of attempts shouldn't be required.
• if any task was not started or not_answered, its Subject Mood should not be required
• move General comments box under the General information section header
