OMN-13215 — OCC Evidence-Source for canonical, provider-swappable delegation ceiling

Central evidence PR for OMN-13215. Provides the Evidence-Source contract + DoD receipts that the deploy-gate / receipt-gate require for the paired feature PRs:

• omnimarket evidence(OMN-11280): bind session_phase_dispatcher contract and receipts #1258 — remove the codex-cli shell-out; canonical HTTP, provider-swappable ceiling.
• omnibase_infra evidence(OMN-12564): refresh checkout metadata proof #2020 — remove the inference.codex_cli operation + codex-cli CLI config; narrow the lane-attestation gate.

dod_evidence

• dod-001-local-verification — focused delegation + routing-authority + inference-effect tests pass, mypy --strict clean, ruff clean, routing-authority gate PASS (12 backends shape-compliant, no cli backends), pre-commit clean.
• dod-002-deploy-readiness-smoke — the HTTP-only inference handler imports with no subprocess/shutil and no _call_cli_backend; the ceiling tier resolves over the canonical HTTP path with no codex binary reference. dev lane only; prod/judge/.201 untouched, no live publish.
• pr-occ-<this> — OCC self-binding receipt for this evidence PR.
