Closes OMN-16490
Follow-up to OMN-16416 (#2021), which fixed session-start.sh:145. This fixes the guard that should have caught it.

The hole

scripts/check_hook_event_names.sh tested:

if ! grep -q 'hookEventName' "$file"; then

That is a whole-file check. Any file with at least one conforming emitter masks every non-conforming one beside it. session-start.sh sets hookEventName at 1533/1542/1546, so the file passed while :145 shipped a hookEventName-less SessionStart envelope — silently dropping additionalContext for every lite-mode session.

The fix

Walk each hookSpecificOutput mention; look for hookEventName within a proximity window; report file:line.

A window is needed because one emission legitimately spans lines in both directions — jq pipelines put hookEventName first and chain .hookSpecificOutput assignments after it, while jq/python object literals open the envelope and set hookEventName later.

Window default is 12, chosen empirically as the smallest value that clears every in-tree false positive. The binding case is lane_termination_guard.py, which builds the envelope at :387 and mutates it at :398. Overridable via HOOK_EVENT_NAME_PROXIMITY.

Keeping it usable

The naive version reported 26 offenders on a clean tree — it was matching prose. Excluded: shell/python comments, python docstring bodies (tracked by triple-quote state, including the opening line), the hook test harnesses, and an inline # hook-event-name: not-an-emission escape mirroring the allowlist-pragma convention already used here.

Known limitation, documented in the script: two emissions closer together than the window can mask each other. Far smaller than whole-file matching, and it fails toward false-negative rather than blocking a correct push.

Two call sites the stricter check surfaced

  • post-skill-delegation-enforcer.sh:88 — emitted {hookSpecificOutput: {}} on its exempt path. A real defect: Claude Code rejects an envelope with no hookEventName. Now emits hookEventName: "PostToolUse", matching the same file's other emission at :105.
  • skip_token_surface_guard.py:271 — pragma added; an argparse help= string that merely names the field.

Verification

  • Guard passes on current dev
  • Reintroducing the :145 regression is caught at that exact line; the old grep -q passes the same file
  • tests/hooks/test_hook_event_names.py — 5/5 green, including its own synthetic negative case
  • Full pre-commit run --hook-stage pre-push --all-files clean (no new findings; the handful of secrets-scanner hits on unrelated pre-existing files — metrics.py, node_golden_chain_sweep_orchestrator/__main__.py, and pre-existing lines 22/68 in skip_token_surface_guard.py far from this PR's line-271 edit — are not introduced by this diff)

Provenance

Recreates the diff from stalled fork PR #2031 (verified byte-identical via gh pr diff 2031). #2031 could not be repaired in place: its branch (awu42/omn-9072-emission-granular-guard) and sole commit both name the ticket this work was originally filed under (OMN-9072), not the ticket it was refiled under after triage (OMN-16490) — tripping the Receipt Gate's identity-binding check — and the PR is from a fork (maintainerCanModify=false), so no fixing commit could be pushed to it. Per repo doctrine (spawn fresh lanes, not raw-ID resumes), this PR replaces #2031, which is being closed with a cross-reference to this PR.

OCC evidence for OMN-16490 is already satisfied by the merged autobind companion onex_change_control#7037 (self-bind receipt occ-self-bind-pr-7037); a superseded manual companion attempt, onex_change_control#7058, is being closed as redundant.

Summary by CodeRabbit

  • Bug Fixes

    • Hook responses for approved delegation scenarios now correctly identify the PostToolUse event.
    • Improved hook-event validation detects mismatched event metadata more reliably and reports affected locations.
  • Chores

    • Refined help output to prevent unintended annotation or emission.
    • Refreshed the secrets baseline timestamp without changing detected entries or configurations.

…me check

scripts/check_hook_event_names.sh tested hookEventName with a whole-file
`grep -q`, so any file carrying one conforming emitter masked every
non-conforming emission beside it. That is how session-start.sh:145 shipped
a hookEventName-less SessionStart envelope while the same file's three
full-mode emissions (1533/1542/1546) kept the guard green. OMN-16416 fixed
:145 and explicitly deferred this guard fix; this closes it (OMN-16490).

The check is now per-emission: walk each hookSpecificOutput mention, look
for hookEventName within a proximity window (default 12, overridable via
HOOK_EVENT_NAME_PROXIMITY), report file:line. The window is required
because one emission legitimately spans lines in both directions (jq
pipelines set hookEventName first and chain assignments after; jq/python
object literals open the envelope first and set hookEventName later).

Excludes shell/python comments, python docstring bodies (triple-quote
state tracked, including the opening line), the hook test harnesses, and
an inline `# hook-event-name: not-an-emission` escape mirroring the
existing allowlist-pragma convention.

Two real call sites the stricter check surfaced:
- post-skill-delegation-enforcer.sh:88 emitted `{hookSpecificOutput: {}}`
on its exempt path -- a real defect, Claude Code rejects an envelope
with no hookEventName. Now emits hookEventName: "PostToolUse", matching
  the file's other emission.
- skip_token_surface_guard.py:271 -- pragma added; an argparse help=
string that merely names the field.

Verification: guard passes on current dev; reintroducing the :145
regression is caught at that exact line (the old grep -q form passes the
same file); tests/hooks/test_hook_event_names.py 5/5 green including its
own synthetic negative case.

Recreated from stalled fork PR #2031 (same diff,
verified byte-identical via gh pr diff), which could not be repaired in
place: its branch and sole commit both named the ticket this work was
originally filed under (OMN-9072) rather than the ticket it was refiled
under after Linear triage (OMN-16490), tripping the Receipt Gate's
identity-binding check, and the PR is from a fork (maintainerCanModify=
false) so no fixing commit could be pushed to it. Per repo doctrine
(spawn fresh lanes, not raw-ID resumes), this PR replaces #2031 rather
than attempting to repair it. #2031 is being closed with a cross-reference
to this PR.

OCC autobind did not mint a companion for this PR: no changed-file candidate could be proven RED against the merge base, and emitting a PR-existence probe instead would be non-falsifiable evidence (OMN-15247). Hand-authored evidence is required.

📝 Walkthrough

Walkthrough

The pull request updates the secrets baseline timestamp, marks a help-text occurrence as non-emitting, emits PostToolUse for an exemption response, and changes the hook-event guard to validate individual emissions within a configurable proximity window.

The validation change can still be bypassed when nearby comments or docstrings contain hookEventName, allowing an invalid hook envelope to pass checks and potentially omit context at runtime. This is a bounded merge-readiness risk that should have explicit owner follow-up.
