fix(tts): prevent double [pause] in xAI auto speech tags for multi-paragraph text

Summary

_apply_xai_auto_speech_tags applied two pause-insertion passes unconditionally:

1. Paragraph breaks (\n\n) → " [pause] "
2. First-sentence boundary → " [pause] "

Both fired on every call, so multi-paragraph input produced a double pause in the TTS audio:

Root cause

The tag-detection guard (_XAI_SPEECH_TAG_RE.search(clean)) only runs once, on the original input text. After the paragraph substitution injects [pause], there is no re-check — so the first-sentence substitution fires even when a pause was already inserted.

Fix

Wrap the first-sentence substitution in a _XAI_SPEECH_TAG_RE.search(clean) guard. If the paragraph pass already added a [pause] tag, the first-sentence pass is skipped. Single-paragraph behavior is unchanged.

Your one-line guard fix was cherry-picked onto main with authorship preserved. Salvage added 3 regression tests (multi_paragraph_emits_single_pause, single_paragraph_still_gets_first_sentence_pause, single_newline_still_gets_first_sentence_pause) so the fix can't silently regress.

One repro note for posterity — the example in your PR body ("Hello world.\n\nSecond paragraph.") actually didn't trip the bug because _XAI_FIRST_SENTENCE_RE has a 12-char floor on the first sentence. The bug DOES reproduce with anything longer (e.g. "Welcome to the demo of our new product line.\n\nIt has many features."), which is what the new tests use. Doesn't change the validity of the fix.
