[None][fix] Responses: round-trip reasoning through encrypted_content - #21058
The Codex CLI calls the Responses API with store: false and include: ["reasoning.encrypted_content"]. It keeps reasoning items in its own history and sends them back on every request with the encrypted_content it received. It sends readable content back only if the item had some. OpenAI's reasoning items carry none, so for a client built against them, encrypted_content is the only copy of the reasoning.
With thinking on, the model did not get its own reasoning back as it wrote it:
The server never wrote encrypted_content. A replayed reasoning item without readable content carried nothing this server could read, so it was skipped (or replaced by its summary).
One assistant turn arrives as several input items: reasoning, message, then one item per call. Reasoning followed by calls was folded into one assistant message, but reasoning followed by text was not, so the turn rendered as two assistant messages.
A template that renders earlier reasoning then renders a prompt that is not the prefix the model generated. DeepSeek-V4.1-Flash renders past reasoning on every turn once tools are declared. Each turn re-rendered the history differently from what the model generated, and prefix-cache reuse stopped at the first replayed turn.
This change:
Writes a reasoning carrier. When include contains reasoning.encrypted_content, every reasoning output item carries encrypted_content = "trtllm-reasoning-v1:" + base64(UTF-8 reasoning text). This holds for the streamed response.output_item.done items and the final snapshot. Both encode the same text, so they carry the same value. Without that include value nothing changes.
Reads it back. A replayed reasoning item with our carrier gets its exact text back as that turn's reasoning. The carrier is preferred to readable content, which a client may have trimmed or edited. Any other encrypted_content (another provider's opaque value, or ours but undecodable) holds reasoning this server cannot read. That item is skipped: its readable text or summary is not rendered as this model's thinking, and the request does not fail. Items without a carrier convert as before (content, else summary, else skipped).
Folds one assistant turn into one message. The items of one turn, in input order (reasoning, then message text, then calls) up to the next non-assistant item, become one assistant message with reasoning, content and tool_calls, as chat completions carries it. Text pieces are joined as they were split. A reasoning item always opens a new message, and text after the turn's calls stays a message of its own, so inputs not shaped like a turn convert as before. History replayed through previous_response_id still drops reasoning, and now keeps the text of a folded turn.
The KV-aware router builds its view of a Responses request with the same conversion, so it sees the same conversation as the worker.
Why the carrier is not authenticated
It is an encoding, not encryption. It holds only the reasoning text, which the same item already shows in content. No ids, request data or server state go into it.
A client can forge one, but that grants nothing new. In a stateless conversation the client already sends readable reasoning content, which the server replays as the model's reasoning, as well as the assistant's messages and calls.
An HMAC with a per-process key would make every carrier unreadable after a restart and on every other replica behind a load balancer. That is the failure this change fixes. A configured shared key would work, but adds a secret to deploy and rotate without making anything safer.
The carrier is not bound to a model. A conversation moved between deployments of different models would replay one model's reasoning as the other's. Keeping a conversation on one model is the router's job, as it already is for readable reasoning. The version in the prefix lets a later format add such a binding without misreading v1 values.
Not changed: when thinking is enabled, and the Harmony (gpt-oss) path, which builds its prompt from its own message types.
Test Coverage
New CPU test file tests/unittest/llmapi/apps/test_responses_reasoning_carrier.py, added to l0_cpu.yml:
Streamed and final reasoning items carry the same carrier, in the documented format. A non-streamed response carries it too.
No carrier when include is absent, empty, or asks for something else.
A replayed carrier restores the reasoning verbatim (edge whitespace, blank lines, non-ASCII) and is preferred to readable content.
A foreign carrier (alone, with readable content, with a summary), a corrupt, altered or non-ASCII carrier, and an item with nothing readable give no reasoning and do not fail the request.
Round trip: emitted by the streaming processor, replayed as the Codex CLI replays it, parsed into the conversation handed to the chat template. The reasoning and text come back unchanged.
DeepSeek-V4.1: for reasoning+text+call, reasoning+call+call and reasoning+text turns, the prompt rendered from the replayed items equals the prompt rendered from the assistant message the model generated. The previous prompt plus the generated reasoning and text is a prefix of it.
Added to existing files:
test_responses_input_preprocess.py: the fold for reasoning+text+call, reasoning+call+call, reasoning+text, and text split across items; items out of turn order keep their own messages.
test_responses_store_replay.py: replay through previous_response_id keeps a folded turn's text and calls and drops its reasoning.
On a build of current main, the new file and the five Responses test files it touches or depends on (test_responses_input_preprocess.py, test_responses_store_replay.py, test_responses_stream_final_consistency.py, test_responses_streaming_tool_calls.py, test_responses_custom_tools.py) pass: 199 tests, against 172 for the five files on main.
Also checked locally, outside CI: Hypothesis properties over the shipped code (carrier round trip, foreign and corrupt values, the fold, stream/snapshot equality over random chunkings, DeepSeek-V4.1 rendering), and a mutation gate in which all 17 planted bugs were caught. The property run found that a non-ASCII payload after the prefix raised instead of being ignored; that is fixed and covered by a test.

Review findings and severity counts are unavailable.
QA Engineer Review
Three unit test files were added, and one CPU test-list file was modified. Tests cover assistant-turn folding, carrier emission and replay, invalid or foreign carriers, and stored-response replay. The reasoning-carrier test is listed in tests/integration/test_lists/test-db/l0_cpu.yml; no manual-QA listing is shown. Coverage verdict: sufficient for the described behaviors. The author reports that 199 tests passed on a build of current main; this result is not independently verified.
Per-File QA Perspective
tensorrt_llm/serve/responses_utils.py: Verify carrier emission only when reasoning.encrypted_content is requested, replay from valid carriers, streamed and non-streamed output, malformed-carrier handling, and assistant-turn folding.
tests/unittest/llmapi/apps/test_responses_input_preprocess.py: Covers assistant-turn folding, tool-call grouping, split text, and cases that start a new assistant message. No integration test-list entry is shown.
tests/unittest/llmapi/apps/test_responses_reasoning_carrier.py: Covers carrier output, replay, invalid or foreign values, and rendered-prompt equivalence. Listed in tests/integration/test_lists/test-db/l0_cpu.yml; no manual-QA listing is shown.
tests/unittest/llmapi/apps/test_responses_store_replay.py: Covers stored-turn replay, including preserving text and tool calls while dropping reasoning. No integration test-list entry is shown.

A client that replays reasoning by encrypted_content alone (the Codex CLI
asks for include: ["reasoning.encrypted_content"] with store: false) got
none back, and a replayed reasoning item without content was skipped, so
with thinking on the model lost its own reasoning every turn. One turn
also arrived as several input items and rendered as several assistant
messages when it had text as well as reasoning.
- With include asking for it, every reasoning output item carries
encrypted_content: "trtllm-reasoning-v1:" + base64 of the reasoning
text's UTF-8 bytes. Stream and final snapshot carry the same value.
- On replay that carrier restores the reasoning verbatim. Any other
encrypted_content (another provider's, or ours but undecodable) is not
replayed as this model's reasoning, and never fails the request.
- A turn's reasoning, message text and calls fold into one assistant
message (reasoning, content, tool_calls). Items out of that order keep
their own messages. Stored history keeps a folded turn's text.

Responses can include a server-prefixed Base64 reasoning carrier when requested. Input conversion can restore reasoning from valid carriers and combine eligible reasoning, assistant text, and tool calls into assistant turns.
Input conversion restores reasoning from valid server-prefixed carriers. It skips unreadable or foreign carriers and preserves accompanying text and tool calls when replaying stored messages.
Eligible reasoning, assistant text, and tool calls are combined into an open assistant turn. Tests cover folding boundaries and compare prompts rendered from replayed inputs.
Reasoning advertised as encrypted can be decoded by recipients of the carrier. Address this confidentiality risk before merging.

Pins the folding for a turn that has no reasoning item: the text of one

…istant message
A reasoning item opens a new assistant message when it is replayed, but one
this server could not read, or one with nothing readable, was dropped without
that boundary, so the assistant text or call after it folded into the turn
before it. Skipping such an item now closes the open message, so the boundary
is the same as for a readable item, minus the reasoning.

Do not emit plaintext as reasoning.encrypted_content.
OpenAI defines this field as opaque encrypted reasoning for stateless client-side preservation and replay. This implementation emits reversible Base64 instead. A client that stores or forwards only encrypted_content under the advertised OpenAI-compatible contract can therefore expose the model's reasoning to any recipient of that carrier.
Replace the Base64 carrier with authenticated encryption and stable server-side key handling that _decode_reasoning_carrier can use. If that cannot be provided, stop emitting this field and document the server-specific limitation instead.

Replace the reversible Base64 carrier in _encode_reasoning_carrier and
_decode_reasoning_carrier with authenticated encryption using established,
stable server-side key handling, ensuring only values the server can
authenticate and decrypt are restored. If suitable key handling is unavailable,
stop emitting reasoning.encrypted_content from _reasoning_output_item and
document the server-specific limitation.

@coderabbitai Thanks. I'm not taking the follow-up about plaintext in reasoning.encrypted_content; the PR description covers this under "Why the carrier is not authenticated".
The carrier exposes nothing the response doesn't already expose. It holds only the reasoning text, and the same reasoning item already shows that text in its readable content. No ids, request data or server state go into it. So anyone who can read the carrier can read the item it sits in. The field name makes no secrecy claim on this server, and the format carries a visible trtllm-reasoning-v1: prefix.
Real encryption would need a key, and the key would bring back the failure this change fixes:
With a per-process key, every carrier becomes unreadable after a restart and on every other replica behind a load balancer.
A shared configured key would work, but it adds a secret to deploy and rotate without hiding anything the item doesn't already show.
Not emitting the field would leave Codex-style clients, which keep only encrypted_content when content is empty, without their reasoning on replay.
For best results, initiate chat on the files or code changes.
@JunyiXu-nv Understood. I withdraw the plaintext follow-up.
The carrier contains reasoning text already exposed in the same item's readable content. Encrypting that duplicate text would not add confidentiality to the response. A per-process key would also break replay across restarts and replicas.
The versioned Base64 carrier serves the client replay requirement you described. I am not requesting a change on this point.
