What does this PR do ?

Fused RMSNorm + residual requires unpacking the output of the layer to extract the output itself and the residual connection. This is already done in transformer_layer.py, but was missing in this file.

MRs are mergable after one approval by either eharper@nvidia.com or zijiey@nvidia.com.
