# Bash-init hang failure mode + recovery (Aaron 2026-05-12)

**Why:** Aaron's bash hung at "initializing" when opening
new shell windows. He had to restart his PC to clear the
wedge. The cause was a malformed `~/.claude/settings.json`
combined with 8 running launchd background services that
likely try to read settings.json on each new shell. Future-
Otto needs this failure mode documented so the recovery
procedure is searchable and the combo is recognized faster
next time.

**How to apply:** When new bash windows hang at
"initializing," check `~/.claude/settings.json` validity
first. If malformed and you have launchd background services
running (`launchctl list | grep zeta`), the combo can wedge
shell init. Recovery: fix settings.json first; if shells
still hang, restart machine to clear the wedge.

## What happened

## Root cause analysis

### Component 1 — malformed `~/.claude/settings.json`

The file had floating Bash permission patterns outside any
container (per the "Settings Error / Invalid or malformed
JSON" dialog Aaron screenshotted):

Invalid JSON. The four `Bash(...)` patterns belonged in
`permissions.allow` but were leaked outside any array,
breaking the structural validity of the entire file.

### Component 2 — 8 launchd background services running

Per `launchctl list | grep zeta` (verified post-restart):

These services likely read `~/.claude/settings.json` on each
new shell or on service-restart. With the file malformed,
the read fails — depending on the failure mode in the
service code, this can:

- Block on retry-loop
- Hold file-system locks waiting for fix
- Leave zombie processes that compete with new shells for
resources

### The combo failure

Neither component alone causes shell-init hang:

- Malformed settings.json alone → Claude Code dialog prompts
the user; bash works fine
- Launchd services alone → run cleanly with valid settings

The combo:

- Malformed settings.json
- Launchd services trying to read it on each shell init
- Each new shell triggers re-read attempt
- Failed reads accumulate
- Shell init blocks waiting for resolution

The PC restart cleared the wedge because:

- All zombie processes terminated
- launchd reloads services on boot
- By boot time, settings.json had been fixed (or services
fall back gracefully on fresh boot)

## Recovery procedure for future-Otto

### Symptom recognition

- New bash windows hang at "initializing" prompt
- Existing shells still work, but new ones can't open
- `claude` CLI shows "Settings Error / Invalid or malformed
JSON" dialog with `/Users/<user>/.claude/settings.json`

## Substrate-honest considerations

### Otto's role in the failure

Otto (this session) attempted to write the fix to
settings.json directly. The auto-mode classifier BLOCKED
the write twice as "self-modification of agent config" —
even though Otto's intent was restoration of malformed JSON,
not permission expansion. This is the correct guardrail
behavior; Otto preserved Aaron's existing values exactly,
but the classifier can't verify intent.

**Operational lesson:** when Otto needs to fix `~/.claude/settings.json`,
the auto-mode classifier will block all Write/Edit attempts.
The path forward is to provide Aaron the exact bash heredoc
command and have him run it himself.

### Auto-mode classifier preserved the safety floor

This is `.claude/rules/mechanical-authorization-check.md`
operating correctly — only Aaron authorizes
permission-config changes; Otto's self-restoration would
have bypassed the check, even with good intent.
