- **#1645 — Razor-cadence Step 5 catch**: `feedback_anti_ossification_discipline_kernels_stay_candidate_not_authority_recursive_application_to_zeta_aaron_2026_05_05.md` was committed via PR #1625 but missed the MEMORY.md index entry. Companion files from same substrate-flow (architecture-provenance, strike-don't-annotate, red-team-three-level, cybernetic-already) were properly indexed. The mechanized razor-cadence workflow (`.github/workflows/razor-cadence.yml`, fires 09:17 UTC daily per B-0192) opened tracking issue #1624 this morning at 10:55Z, surfacing the discipline gap independent of agent-remembering.
- **Memory hygiene audits**: `tools/hygiene/audit-memory-index-duplicates.ts` returned no duplicates; `tools/hygiene/audit-memory-references.ts` returned 455/455 refs resolved. The #1645 index addition introduced no drift.
