Put the cargo-mutants config where the CLI loads it
When a developer runs cargo mutants from this workspace without an explicit --config argument, this root-level file is ignored: cargo-mutants looks for project configuration at .cargo/mutants.toml, as described in its project configuration documentation. No workflow or script in the reviewed tree passes mutants.toml explicitly, so fail = true never takes effect; move the file under .cargo/ or wire it into the invocation.
