Removes the remaining Agent Manager pending-request singleton and scopes request state by instance context. Request lifecycle behavior remains unchanged, including replies, rejections, timeouts, interruption cancellation, and disposal cleanup. Adds focused isolation and lifecycle coverage and removes the stale architecture ratchet entry.
