Changes proposed in this pull request:
Change edit & delete buttons in appointments index to follow same style as the rest of the project. Also swapped edit and add visit to keep same format as the patients index.

Increased width of admit button in patients index to keep the same width as discharge button. This keeps the buttons in line regardless of whether button is admit / discharge.
