Updates to Diagnosis not refreshing the page

Expected behavior:
When I update a diagnosis (make inactive, make secondary, etc.) after clicking the update button on the dialog, I would expect the patient page to show the updates.

Actual behavior:
You need to do a page refresh in order to see the updates to the diagnosis.

Steps to reproduce:

1. Add a new (primary) diagnosis to a patient
2. Save the patient
3. nav away and then back to that patient
4. click on the diagnosis, click on the secondary checkbox, click update
5. Dialog is dismissed, but patient record had not been updated
6. refresh the page to see the updated diagnosis.
