fix: require credible planned-facility project evidence

Result

ADR 0142 planned-facility relationships now require the corroborating project mention to meet the existing PROJECT_MENTION_CONFIDENCE_THRESHOLD. A low-confidence project extraction can no longer admit a high-confidence lw_plans_to_operate relationship.

Minimal fix

• reuse the existing 0.7 project-mention threshold in the shared admission helper;
• add one regression for a 0.4 matching project mention;
• keep unrelated semantic relationships and low-confidence project mentions themselves unchanged.
