Isolated migration candidate — do NOT deploy

This is a merge-only reconciliation of the successful Order Forge image retention patch chain (#61 -> #64 -> #67 -> #68 -> #69) with the independently verified module-only Codex image quota telemetry draft #66. No production code, services, data, or secrets changed.

• Clean Git merge produced a single conflict in .github/workflows/api-route-typecheck.yml because both branches appended separate regression suites. Resolved by preserving all existing image/Firefly/Responses/generation-combo tests and adding the quota telemetry tests.
• All functional source files merged without a conflict. Separate Draft: restore Codex image quota rollover telemetry core #66 telemetry is still only a formatting/rollover module; it is not wired into live image requests. Do not claim final account image quota observability until it is correctly integrated.
• The parent Draft: retain winning image-generation combo artifacts without replaying failures #69 latest focused run 40172923222 completed successfully. The quota-only Draft: restore Codex image quota rollover telemetry core #66 focused run 40171767874 also completed successfully. These do not replace testing the exact combined SHA.
• Exact combined commit 77fac51b1d merges 1e69e0a4bf + 6cc108b64a. The combined branch remains a draft pending its own GitHub Actions run and wider CI.

Must complete before any release

• Run hosted API Route Typecheck on the combined SHA (both regression suites).
• Finish quota sampling on actual selected/failover account and completed-image headers, with no duplicate upstream image request.
• Full off-host typecheck/build and packaging smoke; dependency security gates; end-to-end fake-provider responses; isolated copy of production SQLite migration rehearsal (including legacy migration 170/194 collision), backup/rollback and canary.
• No production deploy without explicit review. All parent PRs remain Draft.
