docs(release_notes): promote v1.104.0 to stable - #2063

Rename release_notes/v1.104.0rc1 to release_notes/v1.104.0 and retarget the notes at the published v1.104.0 tag: stable title, slug and date, the stable Docker and pip versions, the breaking-change baseline moved to v1.103.0, and a stable-to-stable Full Changelog range. Fold in everything merged onto rc/1.104.0 after the rc.1 cut, including the rc.2 session token change, and remove the v1.104.0rc2 page. New breaking callouts cover stdio MCP servers off by default, the proxy exiting when database setup fails, and the session token format for upgrades from v1.103.0 or earlier. A new upgrade note covers the LiteLLM_SpendLogs index migrations becoming no-ops. Update the release notes landing page and move the Auto Router feature history's Coming Next section to v1.104.0.
