Merge downstream efforts

One result of the discussion in/around #168 is that we are concentrating efforts around merging downstream work. There are a number of forks with valuable functionality we need to merge to consolidate development efforts:

• my work on optimism Optimistic Updates, Lazy Dereferences, Refetch queries #199
• @juicycleff's v1 with SQL cache, socket/subscription, and more(?)
• @degroote22's cache read/write

I'm implementing direct cache access ( #176 above) in my next PR.
Most of the differences in juicycleff's fork were now-merged outstanding PRs anyways, and he no longer has an interest with consolidating efforts.
