vim-surround doesn't work properly

Describe the bug / provide steps to reproduce it

With vim-surround, you'd expect ysa") to turn
"Hello, World!" → ("Hello, World!")
but instead, the result is "(Hello, World!)".

Basically, ysa") and ysi") are doing the same thing.

S also doesn't work in visual mode, e.g., viweS" should turn good morning → "good morning" if the cursor is on the word good.
