Using mkksiso on an iso that already has an inst.ks entry is not removing the old one, it just adds a new one to the end. You can explicitly use -r inst.ks but it really should be remove automatically if a kickstart has been passed on the cmdline.
In practice anaconda is using the last inst.ks entry on the cmdline, so this currently isn't causing problems, but it should be fixed.
