Per title - no icon appearing in the toolbar or in additional icons to add to toolbar in firefox 61.0.1.

Same issue here. In fact, I've tried installing Pulsarr 4 times over the last 6 months, and never gotten the icon in the toolbar or additional icons :(

It shows up on a particular movie page. Setup the addon through about:addons
