Does split cat should be working in form creator ?
it's ok when I create ticket, but not in a form.

Ok, I don't remember of that. Indeed, this is a contribution from someone else. If it does not works for you, then probably something broke between 2.11.à and 2.13.9.

Formcreator reached its end of life : native forms will be available in GLPI 11. If you have paid support contact your support provider.
