It would be nice to have an option to tell MarkdownConverter to start every page with a heading so that paragraphs and bullet lists are not split between pages.

I don't believe this is possible.
I know tables have some CSS magic on them that you can enable in order to show the column headers on every page (I think this is even enabled in this extension's default CSS), but I don't know of a similar feature for paragraphs and headings.
I'll leave this issue open for tracking but I don't believe I'll ever get this solved.
