files tmp not loaded and it is store

i am using mongodb-grid_fs to store some images but when i did the post from the form and it did not update, it create a tmp file in the mongo db and i can not access to it.
The only way i can access to it is if i manipulate it in the controller while it do it the post but after that it looks like the cache is save but loss any link with the model.
