constant crashes (d3d11 failed to create swapchain)

Describe the bug

when I am playing (whatever the chamber is) the game crashes suddenly for no reason, no matter what graphics settings I use, the game crashes randomly.

To Reproduce

just play a while and the game crashes

Expected behavior

to let me play without having constant crashes at any time
