Missing translation on Add Contact form

Expected behavior:
Add Contact form should have a tittle

Actual behavior:
Title is missing on Add Contact form. 'Missing translation...' appears instead.

Steps to reproduce:
Go to Patients.
Click 'New Patient'
Scroll down and click 'Add Contact', not the 'Add button'
A modal appears with the contact form.
Check title.
