Adding family member or expense info to social work tab is slow & broken

Expected behavior:
A user should be able to add family member and/or expense information to a patient.
Actual behavior:
When adding a family member and/or expense information to a patient, when you click on Add takes forever and the data doesn't save properly
Steps to reproduce:

1. Edit a patient
2. Go to the social work tab and click on the Add Family Member or the Add Expense button.
3. Fill in the details on the modal dialog that appears
4. Click on Add and wait ... and wait ... and wait....
