[TRACKING ISSUE] Clean up edit patient view design

The edit patient view has a ton of design issues.

• Patient summary styling doesn't match anything in the app (Redesign patient summary section #379; fixed in Form cleanup first pass #385)
• Patient summary doesn't display any helpful hierarchy of information (Redesign patient summary section #379; fixed in Form cleanup first pass #385)
• The tab styling is default bootstrap styling and doesn't match the app (fixed in Form cleanup first pass #385)
• Input sections needs improved spacing and typography (fixed in Form cleanup first pass #385)
• Sections with a section heading, button, and table below need style help
• Call to action buttons in general need help (see [TRACKING ISSUE] Button styles are inconsistent at best, and ineffective and confusing at worst #359)
