Clean up Report Header functions

Not so much a bug...as a requested change.

1. Change the existing code to simply print a part of the header if it is not blank. Else, skip it. For example, if on the Administration > Print Header page the fields Header Line 2, 3 are blank, but Facility Name and Header Line 1 are not blank, print only Facility Name and Header Line 1 on the reports.

2. On the Administration > Header Options page, remove the 5 checkboxes. With the above change, they are not needed.
