Ivory module system improvements

@elliottt has suggested that we could eschew the Ivory module system and compute the dependencies via a traversal of the call/use graph. This would relieve the programmer from tediously including all used definitions in a module.

(Note that in the concrete syntax, we automatically generate a module.)
