Allow crew/dietary form submission through My Area

Instead of sending a google form, it's way easier to just integrate this in My Area. There's a redundant notes form in the Profile model that can be used for this in the api.

So then we're formatting the content in the frontend? Or are we going to add an additional field to the profile which is called Remarks, where people can fill in dietary restrictions and other stuff?
